For many people with disabilities, Home and Community Based Services (HCBS) waivers and the services available through a waiver, help them live in the community, stay in the home of their choosing, find meaningful work, and reach their goals.
There are many services a person could potentially receive through a waiver, based on their needs and goals. Use the Waiver Services Exploration Tool below to find services that you, or someone you support, may qualify for through a waiver. Note: This tool does not include every aspect of eligibility for a specific service and results do not indicate that you will qualify for a particular service. Please talk to your case manager to discuss your specific situation.

24-Hour Emergency Assistance
24-Hour Emergency Assistance is for people who have occasional support needs and may need support available 24 hours.
Details:
This service is for non-911 emergency support
A parent cannot be reimbursed to provide this service
Adult Day Services
Adult Day Services provide structured, daytime programs to meet the health and social needs of older adults in the community.
Details:
This service is only available for people 55 years and older.
Assistive Technology
The Assistive Technology Service provides devices, equipment or technology to help maintain or improve someone's ability to function at home or in the community.
Caregiver Living Expense
Caregiver Living Expense Service reimburses rent and food costs for someone who lives with and cares for the person receiving services.
Details:
The caregiver cannot be related to the person receiving services.
Rent and food costs are reimbursed to the person and not paid in advance.
Chore Services
Chore Services provide assistance and support to help someone continue to live in their own home.
Details:
Chore Services does not cover medical services.
If the person is under 18 years old, there needs to be documentation in the support plan to show why the parent or caregiver cannot do the support received by the Chore Service.
Community Residential Services
Community Residential Services provide training and support for developing, maintaining and improving daily living skills - as well as ongoing residential care and supportive services to adults and children in a licensed setting.
Crisis Respite
Crisis Respite provides short-term care and intervention for a person with specific behavioral or medical interventions and are at risk to themselves or people they live with.
Details:
Support can include an assessment of the situation, developing a plan with providers and caregivers, staff training, and ongoing support to avoid future crisis.
Day Supports Services
Day Support Services, through a combination of help and training, support someone in building life skills, enjoying community activities and be more independent.
Employment Development Services
Employment Development Services can help someone who wants to work, plan and prepare for getting a job.
Details:
This service can help someone who wants to work in the community or start their own business.
Employment Exploration Services
Employment Exploration Services help someone understand and strengthen their knowledge, interests and preferences so they can make informed choices about if and how they want to work.
Extended Home Care Services
Extended Home Care Services are for people who need additional home care nursing, home health aid, home health therapies, or personal care assistance/community first services and supports.
Employment Support Services
Employment Supports is for someone who works in the community or owns a small business and needs on-going support to maintain their work.
Family Residential Services
Family Residential Services provides training and support for developing, maintaining and improving daily living skills, as well as ongoing residential care and supportive services to adults and children in a licensed setting.
Family Counseling
Family Counseling helps the entire family learn how to better understand and assist the person. It can include a number of therapeutic resources.
Family Training
Family Training can help the entire family, through professional instruction, with self-advocacy, person-centered planning and civil rights.
Home and Vehicle Modifications Services
The Home and Vehicle Modifications Services provides modifications for a person's home or vehicle in order to meet a disability-related need.
Home Delivered Meal Services
Home Delivered Meal Services are for people who cannot make their own food and need support for a nutritionally balanced diet.
Details:
Limit of one home-delivered meal a day.
Homemaker Services
Homemaker Services provides assistance with managing general cleaning and household activities.
